Student Concerns
Click on the appropriate button to assist students: The Academic Performance Concern (formerly called Section 3 Notice) form is issued by a faculty member with concerns about a student's academic performance; the Student Concerns Reporting Form can be issued by anyone in the Lehigh community to notify the Care Team of a student's concerning or disruptive behavior.
